Every three years, Centra conducts a Community Health Needs Assessment (CHNA) for each region where our four hospitals are located. The CHNA helps up take a “pulse check” on the communities we serve by helping identify the complex social factors that impact health and people’s ability to make healthy choices. The survey is available until March 31.
